  6. Techniques in semi-quantitative risk assessment.
  7. The task-based approach to risk assessment.
  8. Syndicate Exercise: Working In Small Groups .

Module 2:

  1. What are hazards?
  2. Analysis techniques.
  3. Hazard  identification and analysis through HAZOP.
  4. Syndicate Exercise- Application Of HAZOP To Relevant Processes.
  5. Instilling HAZOP within the risk management system.
  6. Planning and implementing HAZOP actions.

Module 3:

  1. Reliability technology.
  2. Failure Modes and Effects Analysis (FM&EA).
  3. Failures of permit to work systems.
  4. Analysis of potential consequences like mechanics on fire, explosions and toxic releases.
  5. Fault Tree Analysis to explain potential accidents.
  6. Group Exercise on FTA.

 

Module 4:

  1. Human factors and human error.
  2. Hierarchical Task Analysis (HTA).
  3. Application to critical activities onshore and offshore.
  4. Working in small groups on task-based HAZOPs.
  5. Integrating human factor within HSE management system.

Module 5:

  1. Quantified Risk Assessment.
  1. The role of event tree analysis in scenario development.
  2. The role of fault tree analysis for multi-causation analysis.
  3. Applications for ETA and FTA.
  4. Case Study: Working In Small Groups On QRA- Loss Of Containment Event.
  5. Preparation of action plans, planning and implementing.
